This is just a gentle reminder of the policy for those who are registered to volunteer at the 2025 Shelby Touched Twice Clinic who are under the age of 18 and the day of the clinic.
On the day of the clinic, those who are under the age of 18 need to be brought to the clinic by a parent/guardian to fill out the registration form and have it notarized by a notary provided by the clinic. The form states that the parent knows the named child is volunteering at the clinic. It also states the name of the adult that the under-18 child is supposed to spend the ENTIRE clinic day WORKING ALONG SIDE the named adult assisting with the task at hand. The form will only be available on the day of the clinic at the volunteer registration area.
The Volunteer registration form for those under the age of 18 is only available in the Volunteer Registration area the morning of the clinic.
If you are over 18, please download this 2 page paper form printed and signed when you come in to volunteer.
